Payment for Services
You are responsible for all co-payments, co-insurances, and/or deductibles at the time services are rendered.
Please be aware that we charge a convenience fee of $10 payment for any phlebotomy (laboratory) services done in office.
Please also be aware that we charge a fee of $20 payment for any FMLA or disability forms for the physician to complete.
We accept cash, checks, Visa, MasterCard, and American Express. There will be a $35 fee assessed for any checks that are returned unpaid.
Please notify our office if insurance or any other personal information changes.
Know Your Insurance
Please contact us prior to your appointment to ensure we accept your insurance. Your insurance plan has specific requirements and procedures in order for you to avoid liability for full payment on services rendered. It is important that you become familiar with certain information about your insurance plan, including:
- What your plan covers and does not cover
- Whether or not you will need a referral to see our specialists
- Whether the plan offers the option for mail-order prescriptions
- The plan’s benefits and limitations
- Your financial responsibilities, examples: co-payments, co-insurance and deductibles
- What office visits/services/medications require prior authorizations or referrals
Appointments
To make an appointment with one of our physicians please call our office at (954) 620-0011 option 4 and have your insurance cards readily available.
Please plan to arrive 15-20 minutes before your appointment to allow time to complete any necessary paperwork. Or, if you prefer, you may complete any pre-visit paperwork which is available on our website.
Please be sure to cancel appointments within 24 hours’ notice. We reserve the right to charge for any non-cancelled appointments.
Before Your Appointment
It would be helpful if you could bring the following with you to every appointment:
- Your personal identification and insurance card
- Your co-payment or other financial responsibility.
- We collect all payments at check-in.
- Referral, if necessary, from Primary Care Provider or Insurance. Please be sure to confirm with your insurance plan if you require a referral to see a Rheumatologist (Obtaining a valid referral is the patient’s responsibility).
- A list of symptoms or problems you have been experiencing, including how long you have had them and how you are affected.
- A list of current prescription and non-prescription medications, such as vitamins, dietary supplements or herbs you are taking. Please include the frequency and dosage.
- A list of concerns or questions you would like to discuss with the doctor.
- Any pertinent tests such as laboratory tests, diagnostic results, or bone densitometry reports.

Referrals
Unfortunately due to the amount of patients we schedule daily, we are unable to contact patients individually to remind all of them of their referrals. PATIENTS ARE RESPONSIBLE FOR OBTAINING ALL REFERRALS FROM THEIR PRIMARY PHYSICIAN. This includes new patient appointment and follow up appointments. If your insurance plan requires a referral from your primary doctor, you will be responsible for contacting your primary doctor’s office in order to receive your referral. Please contact them at least one week (7 days) before your appointment to ensure your request is processed. Please contact our office before your appointment to ensure your referral is accurate and has been received.
